Hello District 105 Families!
Reminders from the school nursing team-

Students entering Preschool (for the first time-does not apply to students returning to our preschool programs), Kindergarten, and 6th grade require completed school physical exams and immunization records that must be turned in on or before the first day of school (8/21/19). Please make sure your student is ready for school by turning these in now to your school’s office. Required forms can be found at https://www.d105.net/domain/28
The Gurrie/Spring Ave. nursing team will be holding a Nurse Open House at the Gurrie/Spring Ave. nursing office on Wednesday 8/14/19 from 11:30 am-6:00 pm. Stop by to drop off medication supplies with required paperwork, drop off any outstanding health records, and meet one of your school’s nurses.
